The LPN Team Lead – Community Health provides day‑to‑day clinical oversight, coaching, and operational coordination for Community Health Workers (CHWs) and other frontline staff delivering community‑based services. This role blends hands‑on clinical practice with leadership responsibilities, ensuring safe, high‑quality care in home, community, and outreach settings. The LPN Team Lead supports access to health, chronic disease management, social determinants of health interventions, and care navigation. This role serves as a bridge between the community and the health care, government and social service systems, assisting individuals, families, groups, and communities to access resources including health insurance, food, housing, quality care, and health information. Provides informal counseling, support, and follow‑up.

Responsibilities

Serves as the lead clinical support to Physician and/or Registered Nurse.

Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients.

Collaborates with other members of the healthcare team.

Evaluates response to interventions and patient status toward goal achievement.

Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.

Schedule & Travel

Monday – Friday

8am – 4:30pm; but some evenings may be required

Some weekends required

Candidates will be required to travel to Blackford, Delaware, and Jay communities

Qualifications

Associate's Degree, as Licensed Practical Nurse, required.

Requires graduation from a nationally accredited licensed practical nursing program.

Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.

Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.

Successful Completion of Hospital Orientation and Medication courses, required.

1-2 Years ambulatory experience, required.
