Full‑time Path Outreach Specialist – Hampton‑Newport News Community Services Board
Annual Salary: $48,317
Work Schedule: Monday‑Friday 8:30 a.m. – 5:00 p.m. (Winter shelter season alters hours)

In this onsite role in Newport News, VA, you will directly support individuals in need and connect them with essential services, contributing to a stable and rewarding career in the healthcare industry.
Responsibilities
Conduct outreach to homeless client populations living with mental illness or substance use disorders
Perform psychological assessments and develop personalized care plans
Advocate for clients, provide crisis intervention, and supervise medication as needed
Offer daily living support and assistance with basic physical, financial, social, and personal needs
Maintain comprehensive records and collaborate with service providers to ensure holistic well‑being for clients
Qualifications To excel as a Path Outreach Specialist candidate must possess:
Bachelor’s Degree in Human Services (e.g., Social Work, Psychology, Counseling)
At least one year of experience working with homeless populations, or five years of equivalent work experience
Valid driver’s license (to conduct outreach activities and transport clients when necessary)
Strong communication, interpersonal, and organizational skills
Compassionate and empathetic approach to crisis intervention and care planning
Required Knowledge and Skills
Bachelor’s Degree in Human Services (Social Work, Psychology, Counseling) and one year of experience with the target population, or five years of equivalent work experience
Valid Driver’s License
