The Primary Care Physician (PCP) In Our Organization Demonstrates
- Accountability for outcomes: the PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients.
- Coaching for health: the PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with the patient to work toward the goals, and frequently following up on those goals to improve health.
- Simplifying for action: the PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient and when leading the care team toward performance goals.
We are an outcomes‑focused, value‑based organization. For each PCP’s panel of patients, metrics are regularly measured to help PCPs become successful and reach partnership status: patient admissions per thousand, appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work toward those targets with center and market leadership.
Essential Job Duties/Responsibilities
- The PCP independently provides care for patients with acute and chronic illnesses in older adults.
- The PCP takes full accountability for patient care and outcomes, seeking consultation from specialists when needed while staying involved in detailed care.
- The PCP engages with the hospitalist when a patient is hospitalized.
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
- The PCP leads the care team of care promoter, care facilitator, and care coordinator for patients who can come to the office.
- For patients who cannot come to the office, the PCP engages the transitional care team and other resources.
- The PCP has an active role in center management, covers for other providers, and helps recruit patients and additional providers.
- The PCP supervises or collaborates with an Advanced Practice Practitioner and remains accountable for the APP.
- Performs other duties as assigned and modified at manager’s discretion.

- Self‑Care – maintain personal well‑being to care for others.
- Proficiency in Microsoft Office Suite and other software.
- Willingness to travel locally, nationally for training, and regionally for recruiting.
- Fluency in English, verbal and written. Additional language fluency may be required in some centers.
- Use of independent judgment.
- Flexible to work evenings, weekends, and/or holidays as needed.
Education and Experience Criteria
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
- Ability to obtain or hold a current, active State Medical License.
- Board certification or eligibility in the relevant specialty preferred.
- Maintain board certification through MOC, CME, or retaking exams.
- Current DEA number for schedule II‑V controlled substances.
- Basic Life Support certification by the American Heart Association or American Red Cross within 90 days of employment.
