· Provide community-based mental health services to adults through assessment, treatment planning, and intervention implementation
· Collaborate with clients and families to develop individualized, recovery-oriented treatment plans
· Utilize strengths-based and evidence-based practices including Motivational Interviewing, Trauma-Informed Care, CBT, and DBT-informed strategies
· Deliver behavioral interventions that promote skill-building, emotional regulation, and resilience
· Coordinate care with community partners such as primary care providers, psychiatrists, and local organizations
· Prepare for and facilitate PSR group sessions aligned with treatment plan goals
· Maintain accurate and timely documentation of services, progress, and outcomes in accordance with organizational standards
· Participate as an active member of the interdisciplinary treatment team to ensure coordinated care
· Build and maintain professional relationships with clients, families, and key community stakeholders
· Contribute to quality improvement initiatives through data collection, reporting, and participation in team efforts
· Attend required trainings, meetings, and professional development activities

· Must be at least twenty-one (21) years of age to operate a company-owned vehicle
· Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van
· Valid driver’s license with acceptable driving record and current auto insurance
· Reliable transportation required for transporting clients in personal vehicle
· Successful completion of background checks including criminal, driving, abuse/neglect, and fingerprint checks
· Ability to obtain required trainings and certifications to meet DMH standards
· Strong communication, problem-solving, and organizational skills
· Ability to build effective relationships with adults and their families while maintaining professional boundaries
· Knowledge of community resources, case management principles, and confidentiality laws related to behavioral health**Physical Requirements:**
ADA Consideration – Sedentary work. Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects. Requires repetitive movements of hands, fingers, and arms for typing and writing throughout the work shift.
