Up to $35/hour + Performance Incentives | 90% Remote | Bilingual Preferred

SHA, an original Enhanced Care Management (ECM) Hub, is seeking an
experienced Care Coordinator
to support Medi-Cal members across Santa Barbara County.
This role is
90% remote
with occasional field-based work and offers flexible scheduling, a supportive team environment, and the chance to make a meaningful impact every day.
What You’ll Do:
Serve as the primary point of contact for a caseload of ECM members, including individuals experiencing chronic medical conditions, serious mental illness (SMI), substance use disorders (SUD), homelessness, or perinatal risk.
Conduct timely
comprehensive assessments
and develop
SMART person-centered care plans
in line with Medi-Cal ECM requirements.
Provide intensive outreach and
documented monthly engagement
(calls, texts, home visits) to build trust, support goals, and connect members to services.
Coordinate care across settings: hospitals, PCPs, specialists, behavioral health, housing agencies, and more.
Facilitate
Transitions of Care
(TOC) and linkage to Community Supports (e.g., medically tailored meals, housing navigation).
Participate in
Interdisciplinary Case Team (ICT) meetings , quality reviews, and case conferences with supervisors.
Enter complete and timely documentation using Salesforce NPC and internal audit tools.
Required Qualifications:
2+ years
experience in case management, social work, nursing, community health, or a related field.
Familiarity with Medi-Cal populations and health equity.
Strong organizational and documentation skills; ability to track multiple members, deadlines, and goals.
Trauma-informed and culturally responsive approach.
Located in
Santa Barbara County.
Remote Readiness Requirements:
A personal
computer or laptop
capable of running Zoom, Teams, and web-based EMR platforms (e.g., Salesforce NPC).
Fast-speed internet
for reliable access to systems and video calls.
A
quiet, professional background
suitable for making member calls.
Ability to be
on camera for required trainings, team meetings, and ICT presentations
via Zoom or Microsoft Teams.
Preferred Experience:
Enhanced Care Management (ECM), Health Homes Program (HHP), or CalAIM initiatives.
Housing navigation, mental health, or substance use support.
Assisted Living Waiver (ALW), California Community Transitions (CCT), or Community Supports (CS).
Familiarity with
SMART
Goals, care transitions, and red flag indicators.
