Great opportunity within the Greater Downtown Los Angeles area, for a Nurse Practitioner to focus on geriatric, high-risk patients for a private group.

• Board Certified Nurse Practitioner (NP)
• 2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, Palliative Care, and/or Home Health
• Senior Medicare patients with complex needs to prevent hospitalization readmissions and reduce avoidable utilization, conduct Annual Wellness Visits, risk assessments, medication management, manage chronic conditions and coordinate care across specialties
• 75% in clinic and 25% in home visits
• Must have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and preferred to have AWV exp.
• Mon-Fri 8a-5p
• 8-10 patients per day
• Salary based on experience $145-165K with plenty of great benefits and more
