Job Description Job Description ACCENT GROUP AND FAMILY SERVICES JOB DESCRIPTION JOB TITLE: Intensive In- Home Counselor (QMHP-C or T Registration Required- Resident PREFERRED) IMMEDIATE SUPERVISOR: Director of Services DESCRIPTION: Counselors are responsible for a variety of counseling and educational services within the home and community setting. Counselors are also required to develop, coordinate, and implement the treatment goals and objectives of each client, to include ensuring all objectives, goals, and deficiencies are documented and reported accurately and in compliance with organizational policies and guidelines. Counselors will facilitate clients support groups as needed as well as build partnerships with other community organizations. TASKS: 1. Participates in initial client assessment and ongoing reports following the approved format 2. Participates in the service plan 3. Has weekly contact with the Referring Agency 4. Arranges for weekly case supervision with clinical supervisor 5. Acts as a co-leader of parenting and peer education/support groups as needed 6. Acts as the liaison between family, referring agencies, and other community organizations. 7. Coordinates client services between various agencies 8. Maintains case records and documents services rendered 9. Attends weekly staff meetings consistently. Prior approval from management team is required when absent. 10. Coordinates and facilitates staffing for the client: inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision prior to the end of each thirty-day service period 11. Maintains high degree of client confidentiality 12. Works with other staff assigned to cases, communicating effectively 13. Maintains a high degree of professionalism in the community with clients and with referring agencies seeking to build and maintain positive relationships 14. Assists in providing individual and family counseling services to clients in the home and community setting 15. Assists in providing Crisis Intervention as needed REQUIREMENT: Registered with The Virginia Board of Counseling as Qualified Mental Health Professional (QMHP-C or T) Skills: Case management Oral and written communication Group and individual counseling Family Counseling Crisis intervention Report Writing and Other Client Documentation Service Plan Development and Implementation Knowledge of: Community Resources Counseling Interventions CORE Standards Human Development Group Dynamics Computer skills (Windows) Training’s/Certifications: CPR/FIRST AID Medication Training Restraint Training Techniques OTHER: Accessible by phone 24 hours a day / 7 days per week/ 365 days a week Valid Driver’s License Ability to work on call as needed Minimum Qualifications: Bachelor’s degree in a human services field and one year successful work experience providing mental health services to youths ages 4-20 with behavioral issues and developmental disabilities. Must successfully pass a criminal history check by the BIU of DBHDS and a central registry check by VDSS. QMHP-C or T Registration Required with the VA Board of Counseling. Resident PREFERRED.
