Under the supervision of the Team Leader, the Stabilization Home Services Independent Living Coach will provide supervision, guidance, and assistance to people served by Trinity Services. By helping to foster an environment that encourages people to learn and gain independence, the Coach will facilitate personal outcomes for these individuals. The supports and services are provided in a dignified, respectful, and professional manner.

Salary $24.36 per hour

Location New Lenox Area

Reports To Residential Supervisor

Network or Department Network 4- Stabilization Homes Program

Essential Functions

Promotes a living environment that respects those we serve, and always treats them with dignity and respect.

Advocates for the rights of the persons who are served and supported.

Interacts at all times with the people we serve, in a supportive and encouraging manner.

Provides supports and assistance to people to provide an ongoing safe and healthy living environment.

Provides opportunities that will teach and urge people to make choices.

Provides opportunities for people to develop and maintain friendships.

Assist program participants, with planned leisure time activities, assuring age appropriateness.

Ensures that proper medication administration methods and documentation are followed. Assures that people receive the necessary and appropriate medical attention.

Provides transportation that assists people to get to their jobs and/or accomplish personal tasks such as shopping or going to medical appointments. Maintains the vehicle in a safe and clean manner.

Respect the privacy of those that are served.

Be aware of and report all health concerns to the Team Leader.

Consistently implements each assigned individual service plan. Ensures appropriateness of rising and bedtime hours, bathing practices, meals and mealtime settings, and proper sense clothing and attire. Assists persons served on a small group basis, in accessing community activities and events.

Supervises daily routines and activities of persons served; assists persons served with daily grooming skills, laundry skills, room cleaning, meal preparation, yard work and snow removal.

Participates as a member of the Stabilization Home services team in the development and implementation of Individual Service Plans.

Maintains a daily log and exchanges information with other staff.

Utilizes approved verbal de-escalation and physical management interventions during a crisis.

Performs other duties as assigned.

Ability to establish and maintain effective working relationships with persons served, coworkers, and their supervisor.

Ability to maintain professional demeanor in before, during, and after all interactions.

Ability to communicate effectively orally and in writing.

Ability to document work efficiently and effectively.

Time management- the ability to arrive on time for a scheduled shift.

Ability to bend, lift, and carry.

Ability to follow written and oral instructions.

The physical activities for this position involve: Safety Care-approved physical management, balancing, pulling, pushing, standing, climbing, grasping, stooping, pulling, crouching, crawling, kneeling, reaching, lifting, repetitive motion, pushing wheelchairs, and lifting up to 40 pounds.

Physical Demands

Be able to lift over 40 pounds from floor to waist unassisted.

Be able to push and pull 75 pounds.

Be able to complete deep knee squats.

Be able to push wheelchairs up to 200 pounds.

Remain in a stationary position, often standing or sitting for prolonged periods.

Operate motor vehicles.

While performing the duties of this job, the employee is regularly required to stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment.

Physical activities that could be used include: driving, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.

Qualifications

1,000 hours of previous DSP experience working with persons with intellectual/developmental disability.

Letter of references from both a) a person supported previously AND b) a previous supervisor.

A high school diploma or GED is required (Bachelor’s is preferred).

A valid driver's license is required.

Maintain a current CPR, First Aid, and Safety Care card (if required).

Pass a reading comprehension test.

Pass the NADD DSP certification exam within 60 days of hire.
