Job Summary
The Case Manager will collaborate with the Northwest Michigan Coalition (NMC) to End Homelessness and the Michigan Balance of State Continuum of Care (BOSCOC) to implement the Homeless Prevention programs of the Northwest Michigan Community Action Agency (NMCAA). This position involves direct engagement with the homeless population through community shelters and street outreach, assisting clients in securing safe, permanent housing, and connecting them with additional community resources for support. The Case Manager will also be responsible for maintaining client information, notes, and housing plans, as well as completing reports for various programs and grants while advocating for effective solutions to end homelessness.
Responsibilities
Utilize a positive, person-centered approach to create housing-based case plans that leverage individual strengths and resources.
Develop crisis plans, budgeting strategies, and link clients to community resources, including mental health services and financial assistance programs.
Engage with clients to identify housing options, mediate with landlords, and assist in maintaining stable housing.
Collaborate with community partners, shelters, and human service providers to ensure effective access to resources and support.
Participate in case conferencing and coalition meetings to foster coordinated care and outcomes.
Maintain accurate client records in the Homeless Management Information System (HMIS) and complete necessary assessments, such as SPDAT, in a timely manner.
Attend training and meetings as required to stay updated on best practices and program guidelines.
