Sinai Urban Health Institute (SUHI) is hiring a culturally competent Community Health Worker (CHW) II - Clinical with previous health education experience. Under the direction of the department supervisor, the CHW is responsible for providing and coordinating health education and disease management education, assisting individuals with navigating the health and social care systems, and providing case management services (under supervision). They act as a bridge between the patient and the medical system by building trusting relationships with community members served by the hospital or program(s).

This position will focus on pregnant and postpartum patients in the outpatient clinic setting and in Sinai hospital locations. They will navigate patients to perinatal resources, coordinating with SUHI’s other maternal child health CHWs to ensure continuity of care throughout the perinatal period. The CHW is stationed either in the hospital or an outpatient clinic, working directly with patients providing health education and needs assessments, identifying and addressing patient concerns, screening for social needs, assisting with discharge, and providing resources.

The CHW also helps in identifying barriers to accessing quality care, works with individuals to overcome these barriers, and provides referrals. Home visits may be included in future intervention work, and the CHW must be able to work in a fast-paced environment such as the emergency department, when needed.

Position Requirements

Must have high school diploma or GED

2 years of health education experience in maternal child health preferred

Knowledge of Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred)

Passion for and dedication to working with community members and providers

Punctual, reliable and willing to learn

Demonstrated commitment to racial and health equity

Must have a valid driver’s license and vehicle

Proficiency in Microsoft Word and Excel preferred

Bilingual in English and Spanish preferred
