This position reports directly to the Intensive Services Supervisor within the BH division and will perform assessments, recovery planning, resource navigation, referrals to appropriate community services, supported housing, supported employment, and outpatient behavioral health services to adult individuals across the LifePath Systems continuum of care.

The case manager is periodically on call and will respond outside regular business hours. The ACT team meets daily to staff individuals served in an effort to reduce inpatient psychiatric hospitalizations and foster independence. The role connects individuals to primary physical health care, supports applications for qualifying benefits, and promotes choice and autonomy through a strength-based model.

Responsibilities

Perform psychosocial assessments, treatment planning, case management, service coordination, and skills training for individuals with serious mental illness.

Provide crisis intervention and symptom management services and respond to behavioral health crisis situations calmly and respectfully.

Link individuals to primary physical health care, supported housing, supported employment, and community behavioral health services, including crisis services.

Maintain accurate and timely charting, ensuring 90% of service notes are signed within 2 business days and 75% of chart reviews score 80+ on the Golden Thread.

Collaborate daily with the ACT team, Adult Fidelity managers, Program Administrator, and external partners such as local police and mental health professionals.

Respond to community needs, troubleshoot and organize supports, and reduce barriers to improve social determinants of health.

Ensure all required trainings are completed on time and center‑issued equipment is maintained.

Adhere to Texas Administrative Code, HHS Commission regulations, applicable grants, and LifePath Systems policies.

Serve as a bilingual translator when needed for bilingual clients.

Specific Targets

Document engagement and customer service in the electronic health record at least 50% of time worked.

Assess need for client support funds and connect to full array of behavioral health services.

Ensure 90% of service notes are signed within 2 business days.

Achieve 75% of full chart reviews with a score 80+ on the Golden Thread.

Qualifications

Bachelor’s degree in psychology, social work, or related field from a four‑year college or university.

Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention, and symptom management services to individuals with serious mental illness or serious emotional disturbance.

Knowledge of Trauma‑Informed Care Practices required.

CCBHC knowledge and experience preferred.

Experience in Texas Recovery and Resiliency (TRR) preferred.

Must have reliable transportation and a valid driver’s license.

Ability to assess individual needs, coordinate services, and communicate effectively.

Knowledge of the social services system and ability to connect individuals with appropriate services.

Strong multi‑cultural competency and bilingual (English/Spanish) preferred.

Computer proficient, organized, detail‑oriented, and able to write clearly and professionally.
