Job Description Job Description Qualifications Bachelors Degree (Required) Drivers License (Required) Six (6) months of experience providing rehabiltiation services in a volunteer or employment capacity (Preferred) Rehabiliation Duties Provide ethical and culturally-sensitive rehabilitation services to meet patient needs as well as case management services Offer individual and group services in community, office, and telehealth settings Maintain a caseload of PRP (Psychiatric Rehabilitation Program) patients (caseload size TBD in collaboration with program director) Create rehabilitation plans in collaboration with patient and PRP specialist Provide rehabilitation services in accordance with the rehabilitation plan, including psychoeducation about relapse prevention, patterns and behaviors, and treatment processes Complete clinical paperwork (intakes, rehabilitation plans, visit notes) Advocate for patient needs Offer consultation to patients and their representatives Manage medication compliance (with the help of Medical Team) Provide crisis intervention as needed in collaboration with the patient's therapist and PRP specialist Consult with other mental health professionals as necessary Facilitate internal and external referrals to treatment/social services programs, services, agencies as needed Participate in necessary team meetings based on caseload Engage in regular supervision (TBD by PRP Specialist) Administrative Duties Maintain accurate and timely patient charts that includes clinical intakes, progress notes, assessments, treatment plans, and discharge summaries Personnel Duties Complete all necessary HR paperwork Complete at least 12 hours of training per year Physical Requirements Driving Sitting for extended periods of time Standing and walking short distances Lifting (no more than 20 pounds) Carrying/pushing medical equipment Reaching & Grasping Use of senses (vision, hearing, smelling, touching) Listening attentively & Speaking Mental Requirements & Conditions General Intelligence: Ability to learn and comprehend instructions; critical thinking and problem solving Verbal Intelligence: Ability to articulate self verbally and in writing; ability to understand through writing and verbal conversation Numerical Intelligence: Ability to perform basic arithmetic accurately Motor Coordination: Ability to coordinate hands, eyes, and fingers; typing Environmental Conditions & Requirements Noise Conditions: Ability to withstand noises that are typical for an office setting; Ability to withstand noises related to construction work on an intermittent basis (during routine upkeep and maintenance to the office) Injury Exposures: Exposure to mentally-ill and emotionally-disturbed patients; Exposure to sharps tools; Exposure to bodily fluids typical for a medical office setting (i.e., spit through talking) *Other duties as assigned Company Description The Institute for HEALing, LLC (iHEAL) is a mental wellness agency dedicated to holistic health and growth.
