The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state.

The QIDP III is central to the overall responsiveness and effectiveness of an individual’s active treatment program at an SSLC through oversight of the Individual Support Plan (ISP) process, which is centered on the individual’s preferences, strengths, and needs, and includes the creation of the ISP. Responsibilities include coordination, monitoring, documentation, and follow‑up on all programs, services, and plans for the individuals on their caseload, leading and coordinating the interdisciplinary team (IDT), monitoring the active treatment program, determining the need for program revision, identifying and resolving inconsistencies in training approaches, ensuring timely follow‑up for any IDT recommendations, facilitating skill acquisition, and promoting competent interactions of staff with individuals. The role ensures compliance with federal and state standards governing Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) and the Settlement Agreement with the Department of Justice. It requires working with individuals who have complex needs and may exhibit aggressive or self‑injurious behaviors, demonstrating strong organizational skills and balancing competing priorities. The position also requires strong computer skills and the use of an electronic records system, working under limited supervision with significant initiative and independent judgment.

Essential Job Functions (EJFs)

Attend work regularly and may be required to work a specific shift schedule, including rotating schedules, extended shift, overtime, or on‑call status as required.

Orchestrate and oversee the ISP process for all individuals on the assigned caseload, including facilitation and documentation of IDT meetings throughout the year.

Ensure all individuals receive assessments and evaluations as identified by the IDT.

Coordinate and facilitate participation of the individual and, as appropriate, the individual’s Legally Authorized Representative (LAR) in the development and delivery of supports and services.

Ensure follow‑up occurs for any recommendations for supports, services, equipment, or programs.

Observe and interact with the individuals across settings in program areas.

Collect and analyze data on ISP implementation and effectiveness, coordinating with the IDT for necessary modifications.

Train/coaches staff or coordinate training for staff in implementation of services, supports, and teaching methodologies.

Assist in training, coaching, and mentoring of other QIDP staff as needed.

Work in conjunction with behavioral health department staff to integrate behavioral needs into the ISP.

Monitor and evaluate implementation and documentation of ISP plans monthly to assess success and provide recommendations.

Ensure client protections are in place, respond to significant life events, and take immediate action when necessary.

Work within an electronic records system to create and monitor documentation across disciplines, completing all required documentation in compliance with ICF/IID standards.

Perform other duties as assigned, including participation in disaster response or continuity of operations (COOP) activation.

Knowledge, Skills and Abilities (KSAs)

Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities.

Knowledge of discipline‑specific plans such as Positive Behavior Support Plans (PBSP) and Physical Nutritional Management Plans (PNMPs).

Initial Screening Criteria

Bachelor’s degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field such as therapeutic studies, recreation, or a combination of equivalent subject matter.

At least one (1) year of experience working directly with persons with intellectual disabilities or other developmental disabilities.

Additional Information For a related human services field, the transcript must be reviewed for applicable coursework that meets federal guidelines for QIDP designation.

Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE).
