Overview
Habilitative Specialist / Case Manager – Waterloo, Iowa, United States (Hybrid workplace) Apply to join Mosaic as a Habilitative Specialist/Case Manager to coordinate our services and support individuals served. This role involves guiding service delivery, maintaining necessary records, and ensuring compliance with regulatory requirements while delivering our programs with a positive and engaging approach. Travel may be required in/around the Independence, Oelwein, Waterloo, Waukon and Cedar Rapids areas; a agency vehicle is provided for travel. Responsibilities
Become the Behavior Guru: train our staff in behavior modification techniques and provide tools to help them perform effectively. Develop and implement new programs and strategies to support learning, growth, and personal goals for individuals we serve. Maintain records and document success stories of the people you support, in a compliant and organized manner. Coordinate services to ensure all aspects of the program are delivered consistently and in accordance with applicable rules and regulations. Collaborate with the team to create a positive, solution-focused environment and support others in achieving goals. Schedule: Hybrid workdays after training, some evenings and weekends may be required; travel may be required with an agency vehicle provided. Qualifications
Education:
A Bachelor’s Degree in Psychology, Special Education, or a related Human Services field OR 5 years of relevant work experience in lieu of the degree. Experience:
At least one (1) year of direct experience working with individuals with developmental or intellectual disabilities. Compliance Knowledge:
Proven ability to ensure program delivery is in compliance with regulatory requirements. Bonus Points (Preferred Skills)
Setting Experience:
Previous experience in a Home and Community Based Waiver (HCBS) setting is a plus. Soft Skills:
