This position combines two responsibilities: Reporting to the Sr. Director, Implementation & Operations, the Community Health Worker (CHW) supports McKesson Canada’s Trenton Distribution Centre and support INVIVA programs.
Community Health Worker – Trenton Distribution Center (On‑Site) Hours/Week:

Role Overview Acts as a trusted resource for health education, outreach, and navigation of healthcare and social services. Promotes wellness, delivers education, facilitates program enrollment, and helps employees access internal health initiatives. Maintains accurate documentation, fosters partnerships, and upholds confidentiality and integrity.
Specific Role Responsibilities
Provide personalized health guidance and culturally sensitive support to Distribution Center associates.
Promote health literacy and engagement strategies.
Maintain accurate documentation and data integrity.
Help associates navigate healthcare systems, social services, and community resources.
Build relationships with community organizations and participate in outreach.
Support special projects and additional responsibilities as needed.
Working Conditions & Physical Requirements
On‑site, in‑person work in a dedicated private workspace within a traditional distribution center environment.
Primarily computer‑based work with occasional walkthroughs of the warehouse.
Occasional travel for community program collaborations and outreach.
Position Qualifications & Requirements
Minimum 3 years of relevant experience; High School Diploma required. CHW certification or training preferred.
Proficiency in Microsoft Office Suite (Outlook, Excel, Word, Teams, PowerPoint).
Strong interpersonal and communication skills.
Experience in community outreach and health advocacy.
Ability to guide and support individuals in accessing health resources.
Problem‑solving and decision‑making skills.
Highly organized and proactive.
Flexible availability for multiple shifts and occasional travel.
Commitment to privacy, confidentiality, and ethical standards.
Professional communication across all organizational levels.
Adherence to company policies and accurate metric tracking.

INVIVA Program Support (Hybrid/Virtual) Hours/Week:

Role Overview Coordinates with INVIVA RNs, Program Leadership, and vendor partners. Handles patient referrals securely, manages enrollment/unenrollment in vendor programs, assists with reporting, and supports selection of committee activities, without direct patient interaction.
Specific Role Responsibilities
Coordinate with INVIVA RNs, Program Leadership, and Vendor Partners.
Do not interact directly with patients or access private health data.
Process patient referrals securely per protocol.
Support selection committee activities and communications.
Enroll/unenroll patients in vendor programs.
Assist with reports for INVIVA RNs or Program Leadership as needed.
Reporting & Oversight
Program Leadership oversees CHW work, provides resources, and tracks outcomes.
Monthly meetings with Program Leaders and Frontline Leaders to review progress.
INVIVA RNs may assign tasks and provide guidance.
Frontline leadership supports onsite needs.
Work Location
