Millennium Physician Group
Registered Physical Therapist provides skilled Physical Therapy services on an intermittent basis to clients in their homes in accordance with physician orders.
How Will You Make an Impact Millennium Home Care (an affiliate of Millennium Healthcare, LLC) is a leading provider of in‑home health services. As a full‑time Physical Therapist you will help patients regain strength and independence while collaborating with physicians, nurses, occupational therapists, and other healthcare professionals.
Responsibilities
Patient Assessment: Conduct comprehensive evaluations of patients’ physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients’ mobility, strength, balance, and pain management.
Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living.
Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long‑term health and well‑being.
Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.
Requirements
Licensure: Current and valid Physical Therapist license in the state of Florida.
Experience: Minimum 6 months of experience in a hospital, skilled nursing facility, or rehabilitation setting is requested.
Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care.
Professionalism: Strong work ethic, reliability, and a commitment to providing high‑quality care.
Transportation: Reliable transportation to visit patients in their homes.
