Join a mission-driven, New York City based health care organization focused on supporting vulnerable members through compassionate, community-centered care. This role helps members achieve immediate and long-term housing stability by coordinating with public agencies, housing providers, and care management teams. Responsibilities

Conduct phone and field-based outreach to vulnerable members across the service area. Build and maintain relationships with landlords, brokers, and community-based organizations providing housing and social services. Complete thorough interviews to assess member and family-unit needs and determine appropriate supports. Support members in securing housing, including application completion, documentation collection, and interview preparation. Coordinate with providers to obtain required clinical information (including psychosocial and psychiatric evaluations). Accompany members to housing-related appointments when needed. Act as a liaison and advocate with DHS, HRA, ACT, and other relevant City and State behavioral health agencies. Educate members and caregivers on the housing application process, tenant rights and responsibilities, and landlord-related issue resolution. Serve as the housing subject matter expert for the care management team, providing guidance based on case complexity. Attend external stakeholder meetings to strengthen partnerships supporting the overall housing strategy. Expand and strengthen the local referral network and maximize access to available resources and programs. Research potential funding opportunities supporting safe, affordable, accessible housing for the target population. Apply sound judgment and critical thinking to address unplanned issues and complex barriers. Analyze case findings to confirm concrete needs are addressed through appropriate referrals and program activities. Maintain accurate, comprehensive, and timely documentation. Comply with all training, orientation, policies, procedures, and performance evaluation requirements. Perform other duties as assigned by the Manager of Housing Taskforce. Qualifications

Bachelor’s Degree required. 2+ years of experience processing housing applications (examples include NYCHA, HRA, NY/NY). Significant experience supporting clients impacted by homelessness, addictions, mental health needs, entitlements, and housing, including harm reduction and knowledge of key service providers. Bilingual English/Spanish preferred. Proficiency using computers across multiple systems and web-based applications. Working knowledge of tenant rights and responsibilities. Experience working on-site in a behavioral health practice setting. Strong professional competencies: integrity and trust, customer focus, functional and technical skills, written and oral communication. Salary range and/or hourly rate range of $20 - $23.00 is a good faith determination of potential base compensation offered to applicants at the time of this job advertisement and may be subject to modification in the future.
