Bilingual Spanish Medical Social Worker – MSW / LCSW (Conduct Home Visits – Las Vegas / Henderson, NV)
Alignment Health is seeking a bilingual Spanish social worker (MSW required) to join the interdisciplinary Care Anywhere team. The role involves assessing and evaluating members’ needs, guiding families toward resources, and collaborating with a multidisciplinary team to manage physical, environmental, and psychosocial health issues.
Schedule
Monday — Friday, 8:00 AM — 5:00 PM Pacific Time (required).
Home visits: four per day (mileage reimbursement provided).
General Duties / Responsibilities
Conduct telephonic outreach to assigned members to assess health, environment, nutrition, and psychosocial concerns.
Coach and problem‑solve with members to identify and address specific goals supporting health and behavior change.
Provide interventions such as education, community‑based support coordination, and other resources to optimize health and well‑being.
Chart treatments and progress in accordance with state regulations and department procedures.
Refer to case managers or community support services as appropriate.
Provide home assessments for high‑risk members and develop an individualized care plan.
Collaborate with physicians to screen and evaluate members for psychotropic medications.
Understand clinical program design, monitoring, and reporting to better serve members.
Practice as an interdependent member of the health team, providing primary‑health‑care components such as consultation, collaboration, referral, teaching, and advocacy.
Provide crisis intervention, counseling, and discharge or transfer planning as needed.
Consult with hospital staff and administration regarding psychosocial issues and discharge plans.
Develop and maintain relationships with community resources; coordinate with physicians and other service providers.
Partner with RN case managers and the interdisciplinary team on case‑management program implementation.
Experience & Qualifications
Minimum 5 years of experience in care management, assessment, long‑term member or patient care, or community‑based resource delivery.
At least 2 years working with vulnerable adults or the older‑adult population.
Minimum 1 year of experience with motivational interviewing and the ability to apply Motivational Interviewing and Appreciative Inquiry.
Master’s Degree in Social Work (MSW) – required.
Valid unrestricted Social Worker license (LCSW) – required.
Preferred: Crisis intervention training.
Advanced computer skills with Microsoft Word and Excel.
Strong written and oral communication; ability to maintain constructive relationships with diverse stakeholders.
Problem‑solving, organizational, and time‑management skills; ability to prioritize multiple tasks in a fast‑paced environment.
Mathematical skills to perform calculations and basic statistics; reasoning skills for advanced problem solving.
Training & Specialized Skills
Effective communication, leadership, teaching, and collaborative skills.
Understanding of complex care‑management principles.
Ability to build trust with members, clients, and patients.
Knowledge of current and potential member needs to support health and well‑being changes.
Work Environment & Physical Demands The role involves traveling for home visits and may require standing, walking, sitting, and manual handling. Employees may need to lift or move up to 10 pounds. Reasonable accommodations are available for individuals with disabilities.
Pay Range $72,452.00 — $108,678.00 (based on location, education, responsibilities, and experience).
