Description New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24‑hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. Peer Specialist

Responsibilities

Provides peer support services either in office or in community for an assigned group of clients monitoring the activities of Care Coordination for high utilizing clients of New Horizons services

Assists the Care Coordination Team with activities for assigned clients

Prepares written documentation for each service conducted

Provides transportation to consumers as needed

Requirements New Horizons will need you to have the following qualifications:

Certified as a Peer Specialist or in process of Certification preferred

Valid and current Florida Driver's License required with no more than 6 points in the past 5 years
