Responsibilities
Ensures consumer and guardian participation in the development of service plans and personal futures plans.
Coordinates development of each person served’s personal futures plan and schedules team meetings (times, dates, locations, etc.), informing all team members.
Schedules and chairs screening for program vacancies and develops a waiting list for potential consumer vacancies.
Monitors all service sites to ensure delivery of services in accordance with contractual, legal, and regulatory requirements; implements/coordinates necessary plans of correction from regulatory bodies.
Monitors all incident reports to address patterns of incidents, shares reports with appropriate external agencies (area programs, social services) when necessary; investigates incidents of abuse, neglect, or mistreatment in a timely and thorough manner; initiates plans of correction to prevent recurrences.
Monitors, implements, and supervises delivery of service plans and personal futures plans and staff training; ensures all written training programs are implemented and revised as needed; documents consumer progress or regression; initiates discharge planning when appropriate; maintains records appropriately.
Maintains fiscal spending within approved budget limits, including household expenses, repairs, vehicle expenses, personnel costs, and consultant services.
Monitors workers’ compensation and unemployment claims for assigned service sites; proactively reduces claims and minimizes agency risk/exposure.
Other duties as assigned.
Qualifications
BA/BS in Business, Health Care Administration, or Social Services. A high school diploma or GED equivalent and three years of related job experience may substitute per area regulations; exceptions must have written approval from Regional Management.
At least two years of verifiable work experience providing services and supports to individuals with developmental disabilities.
One year of prior supervisory experience preferred.
Working knowledge of federal, state, and local regulations concerning services to individuals with intellectual or other developmental disabilities.
