A mission-aligned behavioral health organization is seeking a
Tailored Care Management (TCM) Extender
to support care coordination and whole-person health services for Medicaid beneficiaries. This is a full-time, in-office role based in Greenville, NC, ideal for professionals with a background in behavioral health, care navigation, or community-based services.
As a TCM Extender, you will play a vital role in supporting the delivery of North Carolinas Medicaid Tailored Plan services ensuring that members receive coordinated, compassionate care that reflects both clinical needs and social determinants of health.
Key Responsibilities:
Assist Care Managers in coordinating services, scheduling appointments, and following up with members
Support documentation and communication across medical, behavioral, and community-based providers
Help members navigate Medicaid Tailored Plan benefits and access key health and social resources
Monitor member engagement and alert the clinical team to potential risks or barriers
Maintain accurate records and support compliance with Tailored Care Management requirements
Provide culturally responsive, person-centered support to diverse populations
Qualifications:
Associates degree in Human Services, Social Work, Psychology, or related field (Bachelors preferred)
Experience in behavioral health, care coordination, or case management strongly preferred
Working knowledge of Medicaid, social service systems, or community resource navigation is a plus
Strong communication and organizational skills
Ability to work well in a team-based, fast-paced environment
