Job Summary and Responsibilities

Performs screening and identifies needs related to the patient’s plan of care, providing referral information and education; may assist with placement into nursing homes, treatment centers, or other venues for appropriate care and offer episodic counseling related to identified active issues.

Communicates social, emotional, and patient/family stressors to interdisciplinary healthcare provider teams in relation to the patient’s plan of care and advocates for advanced care planning.

Assists in monitoring and trending patient outcomes to validate the effectiveness of the care plan and services provided.

May meet patients and/or family members in the community, at home, or other locations; must assess the environment for safety and, if necessary, elevate concerns to the medical social worker, licensed social worker, and/or registered nurse team member for further action.

Researches and identifies community resources, vendors, medical supply companies, healthcare agencies, and other resources appropriate to the patient’s individualized plan of care.

Job Requirements Required

Master’s degree in social work

2 years of relevant experience or advanced degree required

Arizona: Licensed Master Social Worker (MSW:AZ)

Ability to utilize psycho-social counseling skills in a focused and limited manner to support episodic crisis intervention for patients with complex psycho-social involvement

Excellent customer service and presentation skills are a must

Strong interpersonal and written communication skills are essential

Demonstrated ability to apply analytical and problem solving skills

Demonstrated ability to manage multiple tasks or projects effectively

Ability to work independently as needed with a high degree of detail orientation

Ability to work collaboratively with an interdisciplinary care team

Excellent computer skills and ability to learn new systems

Strong organizational (time management) and interpersonal skills

Ability to handle multiple priorities with strong attention to detail

Ability to communicate effectively using written and verbal skills; proficient in email communications and internet usage, along with basic use of Google Office

Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost)

Ability to work autonomously within a matrix environment without direct supervision or support

Preferred

3 to 5 years of experience preferred
