Job Title: Social Worker (MSW) /Case Manager (LCMSW)
FLSA Status: Exempt
Sign-On Bonus: none; no relocation package is currently offered
Location: Senior TLC Center – Shelby, NC
Position Overview Senior Total Life Care (STLC) is seeking a dedicated and experienced Social Worker / Case Manager to provide direct case management services to participants in a home and community-based care model. This role includes participant screening, counseling, care coordination, and resource linkage. The position requires regular in-person visits to skilled nursing facilities (SNFs) and private homes across Gaston, Cleveland, and Lincoln Counties.
Key Responsibilities
Participate in initial assessments, care planning, and biannual reassessments of participants as part of the Interdisciplinary Team (IDT)
Maintain accurate and up-to-date case management records
Provide individual and family counseling; develop and lead group counseling sessions and therapeutic activities
Advocate for participant access to services and facilitate referrals to community agencies
Serve as a liaison between participants and external organizations
Advise staff on the impact of social, emotional, and cultural factors on health care delivery
Foster team cohesion and effective staff relationships
Contribute to the development of social work programs and policies
Assist in coordinating 24-hour care delivery
Document participant status changes and communicate updates to the IDT
Update care plans throughout the reassessment cycle
Collaborate actively with the IDT to ensure integrated, person-centered care
Promote team unity and effective communication across discipline
Implement SMART goals and interventions in participant care plans, ensuring timely documentation per organizational standards
Uphold Senior TLC’s mission to support seniors in maintaining independence and quality of life in the community
Perform other duties as assigned
Skills And Competencies
Proficiency in individual therapy techniques for geriatric populations
Strong analytical thinking and problem-solving abilities
Ability to connect clients with appropriate community resources
Effective communication and collaboration within interdisciplinary teams
Commitment to ethical practice within scope of licensure
Ability to meet standardized competencies as defined by Senior TLC and CMS
Team building and relationship management skills
Documentation and recordkeeping proficiency
Cultural sensitivity and understanding of psychosocial factors in healthcare
Flexibility and adaptability in a dynamic care environment
Desired Qualifications
Minimum of three years of experience in health and human services, with emphasis on geriatric care
At least one year of direct work with frail elderly individuals in community or clinical settings
Proven background in interdisciplinary care planning and coordination
Strong foundation in geriatric counseling and linking clients to appropriate community resources
Education, Licensure, Registry And Certification
Minimum: Bachelor’s degree in social work
Preferred: Master’s degree in social work from an accredited institution
LMSW, LISW, LCMSW, or LCSW
Additional Requirement: Valid North Carolina Driver’s License and access to a personal vehicle
Health Requirements
Must be medically cleared for communicable diseases and up to date on immunizations
Must be able to provide care to adult and geriatric populations
