Job Summary
We are seeking a compassionate and skilled
Hospice Medical Social Worker
to join our interdisciplinary care team. In this vital role, you will provide emotional support, counseling, and resource coordination to patients and families facing terminal illness. You will work closely with the patient’s care team to ensure that their psychosocial, emotional, and spiritual needs are met during their end-of-life journey. You bring empathy, clinical insight, and a strong sense of advocacy to your work. You are also a self-starter who values autonomy while embracing the collaboration that hospice care requires. Key Responsibilities
Deliver social work services in accordance with hospice standards of care and regulatory requirements Conduct comprehensive psychosocial assessments related to the patient’s illness, family dynamics, and home environment Participate in the development and ongoing review of the patient’s individualized Plan of Care Provide counseling and support to patients, caregivers, and families throughout the dying process and into bereavement Identify and address emotional, social, cultural, and financial needs that impact the patient’s quality of life Assist families in planning for care needs, including advanced directives, long-term care, or funeral arrangements Facilitate connections with community resources, financial assistance programs, and support groups Document all interventions, evaluations, and communications in accordance with agency policy and regulatory standards Participate in interdisciplinary team meetings and provide insight into psychosocial factors affecting care Qualifications
Master’s degree in social work (MSW) from an accredited program Minimum of one (1) year of experience in healthcare; hospice or palliative care experience preferred State licensure as required (LMSW, LCSW, etc.) Valid driver’s license and reliable transportation to perform home visits Knowledge of hospice philosophy, grief processes, and end-of-life care best practices Excellent communication, counseling, and organizational skills The above statements are only meant to be a representative summary of the major duties and responsibilities performed by the employee in this position.
