$5,000 Sign on bonus may be available for qualified candidates * Competitive Wages * Benefit package for eligible employees * The Master of Social Work (MSW) Case Manager plays a pivotal role in maintaining the quality of care patients receive during medical centerization and post discharge while ensuring the organization is fully reimbursed. The MSW Case Manager (CM) must have a high level of expertise and skill to create effective strategies for short and long-term goals to prevent readmissions and address unpredictable situations. They must be proficient in planning, executing, and monitoring the effectiveness of care provided, ensuring timely transitions across the continuum of care. This includes educating and consulting with physicians and the healthcare team to ensure accuracy of medical necessity criteria and appropriate level of care. Skills in conflict resolution, decision-making, and team building are essential. The MSW CM practices according to care management processes, utilizing tools, standards, models, goals, and performance improvement concepts. They provide relevant clinical data to outside agencies to ensure compliance and represent the organization professionally. The role also involves assessing patients’ psychosocial and spiritual needs, providing education and counseling. Responsibilities

Provide social work intervention related to hospitalization, including crisis management, healthcare decision-making, illness adjustment, ethical/legal concerns, discharge planning, transitional care, child or elder abuse, domestic violence, competency, financial problems, compliance issues, substance abuse, mental illness, and other psychosocial barriers. (30%)

Collaborate with healthcare team, patient, family, and significant others to transition the patient to appropriate care levels, advocating and negotiating with an emphasis on self-determination. (20%)

Work with physicians on competency/capacity assessments, legal guardianship, involuntary psychiatric admissions, and other legal/ethical matters, completing necessary documentation. (20%)

Collaborate with the Care Management Team to provide information on admission, stay, discharge, managed care, reimbursement, and regulatory standards. Other Duties and Responsibilities

Complete psychosocial assessments, facilitate family meetings, and coordinate resources. Analyze clinical information to ensure quality care and appropriate location of services. Negotiate patient status with payers and facilitate discharge planning. Participate in care coordination to identify high-risk factors and ensure documentation. Build relationships with patients, families, and facilities, providing excellent customer service. Maintain professional development and licensure requirements. Perform all other duties as assigned. Qualifications

Master’s Degree in Social Work (MSW). Experience in casework principles, crisis intervention, transition planning, and community resources. Knowledge of medical social work practices, with preferred experience in healthcare settings. Adherence to NASW Code of Ethics. Competency in Microsoft Windows; membership in professional organizations preferred. Physical/Mental Requirements

Ability to handle stressful situations, wear PPE, move freely, and perform physical tasks as needed. Employment Details
