Job Details The Lead Social Worker provides comprehensive social work services, assesses patients' and significant others' ability to cope with physical, emotional and environmental challenges, and collaborates with the multidisciplinary healthcare team to establish discharge plans. The role includes:

Collaborate with the healthcare team to design and implement discharge plans.

Assist departmental leadership with clinical expertise and knowledge.

Coordinate community‑based referrals to support patients' continuum of care.

Facilitate team and family conferences when necessary.

Ensure completion of placement documentation for nursing home grants and applications.

Document all relevant information in line with hospital policies and procedures.

Provide psychosocial counseling and crisis interventions, including child abuse/neglect, domestic violence, elder abuse, guardianship, foster care, adoption, mental health placement, advance directives, trauma and substance abuse.

Educate patients and families on health insurance and welfare application processes.

Supervise field placement of MSW students and mentor new staff members.

Participate in daily staffing coverage, ensuring weekend and holiday coverage.

Attend daily patient safety huddles and follow up on identified concerns.

Communicate information to Manager/Director and serve as a resource for discharge‑planning issues.

Work Commitment:

Floats across three New Jersey locations: Cherry Hill, Washington Township & Stratford.

1 weekend shift per 4 weeks.

Major holiday commitment.

Education and Experience

Graduate from an accredited Master's of Social Work (MSW) program.

Minimum 2 years of hospital‑based social work experience.

Certifications

Active LSW or LCSW required for New Jersey positions.

Preferred LSW or LCSW for Pennsylvania positions.

Salary Range $57,366.99 to $81,747.48 per year (actual rate based on experience, skills and qualifications).

Work Shift Workday Day (United States of America).
