Posted Tuesday, September 9, 2025 at 4:00 AM About River Edge Behavioral Health River Edge Behavioral Health serves more than 10,000 Georgians and offers outpatient behavioral health services at our Emery Highway (Macon), Monroe County, and Baldwin County locations. Outpatient services include behavioral health, nursing, psychiatric assessment, stabilization and medication maintenance, crisis intervention, and related awareness, education and recovery programs. The Permanent Supportive Housing Program serves Fulton County residents, including primary tenants and family members. The program uses Housing First and Harm Reduction models to help people with disabilities live more stable, productive lives. It supports individuals who are homeless, have very low to no income, and/or face serious challenges such as substance use, mental illness, HIV/AIDS, or other barriers to a successful life. Schedule:
Monday–Friday 8:30 AM–5:00 PM FLSA Classification:

Provide clients with training in personal hygiene, accessing community resources, medical adherence, household management, or other needs as outlined in the client’s Individual Service Plan. Ensure that client interactions are person-centered and goal-focused. Perform home visits and off-site case management meetings. Provide referrals for comprehensive services to ensure housing sustainability. Maintain a caseload of 15–20 individuals. Maintain contact with all clients based on their service needs level. Identify and provide intervention for potential crisis situations. Assist with securing supportive services, including mental health, primary healthcare, and substance abuse support. Facilitate relationship building between landlords, caretakers, and clients. Provide transportation to appropriate community-based resources. Communicate and collaborate with outside agencies and referral resources, including food stamps, Social Security, and disability benefits. Enter participant information into the database in a timely manner and update as needed. Complete client entries, exits, and self-sufficiency matrixes. Maintain complete and organized files. Maintain and provide documentation on program goals and outcomes. Maintain appropriate case documentation and client confidentiality. Prepare and submit reports to the Supervisor or Team Lead as required. Other assigned duties. Qualifications
Certified Peer Specialists and individuals with lived experience of homelessness, substance use, and/or mental health diagnoses are strongly encouraged to apply. REQUIRED Bachelor’s degree in a social service field (Social Work, Sociology, Psychology, Human Services) or related field. 1 year of experience in case management is strongly preferred. 2 years’ experience working with individuals previously experiencing homelessness in a community setting preferred. 2 years’ experience working with individuals with substance use and/or mental health diagnoses preferred. Knowledge of Housing First and Harm Reduction models preferred. Proficiency with the Homeless Information Management System, Client Track, preferred. Reliable transportation and a valid Georgia state ID, required. Empathy: Ability to understand and share the feelings of others, crucial for providing effective peer support in challenging situations. Communication: Strong verbal and non-verbal communication skills to facilitate meaningful connections with peers and convey information clearly. Case Management: Proficiency in organizing and managing client cases, including coordinating services, maintaining records, and tracking progress. Crisis Intervention: Skill in providing immediate support and assistance to individuals experiencing mental health crises, ensuring their safety and well-being. Documentation: Ability to accurately record client interactions, progress, and treatment plans in accordance with organizational standards and confidentiality protocols. Additional Information
