Shelter and community-based services meet survivors where they are to help them discover their strengths and regain their freedom. POSITION SUMMARY Under general supervision, provides direct covered services to individual clients and coordinates with community support and stakeholders. Responsible for managing appointment schedules, conducting testing assessments, facilitating referrals, and participating in ongoing learning experiences. MINIMUM QUALIFICATIONS Bachelor’s degree from an accredited college in a field related to case management, such as social work, nursing, public health, or other human services-related fields. Or High School Diploma and/or GED plus four years of combined volunteer and work experience comparable to professional knowledge. Skills and abilities that document experience in working with HIV-positive individuals or providing HIV/STD testing to high-risk individuals. Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STDs. Able to work a flexible schedule on weekdays and weekends as needed per program requirements. Bilingual: English/Spanish skills. Must be able to communicate with Spanish-speaking clients; considering the importance of knowing & communicating medical terminology in both languages. Must complete the agency’s orientation and demonstrate competency in skills and knowledge per R9-20-204 (Article 2: Misdemeanor Domestic Violence Offender Treatment / Application or Renewal Approval Process). Have good organization and time management skills. Effective January 3, 2022, all CPLC Employees and all Service Providers are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, weekly COVID-19 testing at their expense, and other requirements established by CPLC from time to time.
