Host Home Provider – Fort Collins, CO
Support Inc. is a highly regarded service agency serving individuals with intellectual and developmental disabilities. We provide the highest level of care and support for our clients and host home providers.
Overview We are seeking experienced Host Home Providers willing to open their home and foster an individual who can no longer live independently in their own community and prefers not to live in a communal setting. As a Host Home Provider, you provide a private bedroom and hands‑on support and ongoing daily care.
Responsibilities
Provide personal care including hygiene, meal preparation, and dressing.
Assist clients with medical appointments and medication administration.
Provide transportation as needed.
Offer employment coaching to help clients secure jobs.
Plan and participate in recreational activities to enhance social development.
Include clients in family life and activities and accompany them to community venues.
Document progress notes and medical records.
Train clients on life‑learning skills.
Qualifications
High school diploma or equivalent (Required).
At least 1 year of experience as a host home provider.
Complete and pass criminal and motor vehicle background checks.
Training in Medication Administration, CPR/First Aid, and maintain ongoing training requirements.
Preferred experience with behavioral and medical needs (e.g., CNAs, LPNs, RNs).
