DIRECT CARE WORKER, HCBS (HOME & COMMUNITY BASED SERVICES) Tyler's Place AZ is a family-owned agency based in Chandler, AZ licensed throughout Arizona. We are a rapidly growing agency that seeks providers to work with individuals with developmental disabilities in the home and community. As a Direct Care Worker, you will aid and support all age groups to meet their individual goals. We offer a fun and easy-going environment with a strong work ethic and are looking to add to our dynamic team. Responsibilities
Respite (RSP): supervise and care for Tyler\'s Place AZ members; support their physical, mental and/or emotional wellbeing in their home or community while relieving a family member or caregiver. Attendant Care (ATC): assist Tyler\'s Place AZ members in creating/maintaining a safe, clean living environment; may include personal cleanliness; help members engage in community outings and activities; help develop and maintain a daily schedule or plan of care. Habilitation (HAH): assist Tyler\'s Place AZ members in a variety of interventions based on the members\' specific ISP goals to help them become more independent by improving their daily life and to be a valued member of their community. Interventions may include Habilitation therapies, behavior interventions and/or sensory motor activities. Qualifications
Must pass AZ DPS Level 1 Fingerprint Clearance application & Criminal History check - State of Arizona requirement Valid Arizona Driver\'s License with clean driving history - State of Arizona requirement (Willing to transport members) CPR & First Aid Certification (Preferred) Article 9 Certificate (Preferred) Direct Care Worker Certificate (Preferred) Prevention & Support Certificate 18+ years of age Reliable Transportation High School Diploma or GED Experience working with individuals with developmental disabilities or Authorized to work in the US Authorized to work in the US Job Details
Job Types: Full-time, Part-time Pay: $17.00 - $20.25 per hour Benefits: 403 B Retirement; Dental insurance; Flexible schedule; Health insurance; Life insurance; Paid time off; Professional development assistance; Vision insurance Schedule: 10 hour shift; 12 hour shift; 4 hour shift; 8 hour shift; Day shift; Evening shift; Monday to Friday; Morning shift; No nights; On call; Rotating weekends; Weekends as needed; Weekends only Location: Openings all over Arizona
