Job Overview The Registered Nurse Care Manager provides comprehensive, patient-centered care management for older adults enrolled in a community-based care program. This role delivers high-quality clinical assessment, individualized care planning, implementation of services, and ongoing evaluation to ensure safe, medically appropriate, and cost-effective care.
Responsibilities Coordinating interdisciplinary team services, managing participants recently discharged from hospitals or skilled nursing facilities, supporting chronic disease management, collaborating with social workers and physicians, and gathering medical and psychosocial data to guide care decisions. This RN plays a key role in optimizing outcomes, preventing avoidable hospitalizations, supporting functional independence, and ensuring seamless care transitions for geriatric and medically fragile participants.
