Summary
The Senior Living Lead Physical Therapist is responsible for the day-to-day clinical care of patients on HH and Outpatient Services. Process orders and attend clinical meetings within the facility. Responsible for operationalizing all components of the SL plan in their facility/facilities. Reports to SLD for all operational, growth, quality and compliance matters in their facility/facilities.
Responsibilities
Documents all Outpatient and HH visit types per company standards.
Timely initiation of care for Outpatient and HH SOC.
Schedules patient at appropriate time for therapy.
Identifies patient and family education needs.
Collaborates with the team to establish interdisciplinary team goals.
Utilizes equipment in a proper and safe manner, reports broken or faulty equipment.
Follows medical regimen developed for the patient. Is aware of and adheres to precautions (THR, TKR, Cardiac, Wound, etc.).
Utilizes the following safe treatment techniques: transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, etc.
Utilizes treatment approaches according to established PT protocols and policies.
Utilizes alternatives or other innovative approaches as needed.
Processes and secures physician orders.
Aware of financial situation and plans care appropriately; ensures that patient is aware of home/work place accessibility; ensures patient awareness of needed equipment, and makes plans to secure it; designs a home exercise program to address all pre-existing problems; makes all appropriate referrals at D/C.
Serves as a member of the Performance Improvement team
Actively participates in Department Team meetings.
Conducts in services for other staff members as needed or identified.
Participates in weekend and holiday rotation as needed
Participates in student supervision as indicated.
Assists in the interview process for new therapy department employees as needed.
Ensures all therapists perform treatments and documentation that meet all company standards.
Attends Team Conference weekly and functions as an active participant of the ID team.
Proficient in Wound care as allowed by state practice acts.
Education and Experience
Must be a currently licensed Physical Therapist
Recent graduates who have applied for the registration exam may be considered for provisional appointment.
CPR certification
Membership or certification in national association (as applicable)
Must be willing to accept management duties as assigned
