Individual Home Support
The Family Advocacy and Community Education Center is seeking a caring, responsible individual to join our Individual Home Support team. The IHS staff member implements individualized programs as specified in Individual Service Plans, provides for individuals’ health and welfare, and ensures general cleanliness and safety of individuals’ homes.
Pay Rate $18.75 / Hourly
Varied Hours
Essential Functions
Instruct and assist individuals with personal hygiene, housekeeping, social skills, leisure time use, advocacy, and response to emergency situations.
Facilitate community integration by encouraging individual preference and providing opportunities for recreation activities.
Encourage and assist individuals to pursue relationships and develop rapport with local community resources such as banks, physicians, stores, etc.
Instruct, assist, and teach individuals meal planning, preparation and grocery shopping; and encourage individual participation to the fullest extent.
Monitor individuals’ medical health, assist in making doctor appointments, provide transportation as needed, and advocate for individuals when necessary.
Deal with immediate problems in the home; notify direct supervisor or the on-call supervisor.
Assist individuals with financial management to the degree specified in the Individual Service Plan; and manage petty cash, including documenting and providing receipts for all money spent.
Instruct and physically assist individuals with housekeeping activities on a daily and weekly basis; and promote a sense of pride and ownership in the home.
Ensure home is safe and free of clutter and debris; and instruct individuals in fire prevention and evacuation procedures.
Administer medication and/or monitor medication use for individuals, and document in accordance with Department of Public Health policies and procedures.
Ability to provide transportation with Agency or personal vehicle as required by supervisor.
Implement and document individual behavior programs where applicable.
Assist and/or support individuals in emergency situations.
Implement crisis management as needed and implement PBS consistently.
Work assigned schedule that may include weekends and/or holidays.
Comply with reassignments and directions from division Supervisor or Director.
Work modified schedule when individuals’ day programs are closed (site specific).
May be required to use employee’s personal vehicle for work purposes.
Perform other duties as assigned by the supervisor, division management team member, or division director including but not limited to working weekends and/or holidays.
Participate in all staff meetings and required training sessions, including in‑service training, as scheduled.
Maintain all program and Agency required trainings.
Qualifications
High school diploma/GED preferred.
Skill obtained through Human Rights, Fire Safety, First Aid/CPR, MAP, Positive Behavioral Supports, and any site‑specific certifications.
Ability to assist and/or support individuals as needed and in emergencies that require full physical assistance.
Ability to transport individuals legally and safely as needed.
Ability to physically restrain individuals exhibiting unsafe behavior.
Ability to follow policies, procedures, and directions.
Ability to work and communicate with others.
A qualifying criminal background check, driving record and a valid driver’s license are required.
Ability to provide current copy of TB assessments, and then every two years.
