OBJECTIVE:
Provides Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) services to residents in accordance with the Individual Service Plan (ISP).
Assistance includes either physical assistance or standby supervision.
Provides support to new resident assistants.
May provide some laundry and housekeeping tasks.
May act as shift leader as established by community standards.
QUALIFICATIONS:
Minimum Qualifications: Must demonstrate competency in the following areas:
High school diploma or general education degree (GED), or three (3) months related experience and/or training or equivalent combination of education and caregiver experience desired.
Current Arizona State Caregiver and Med Tech Certification.
CPR and First Aid certification or achievement within 30 days of employment if required by State Standards.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Assist resident with all personal care skills (bathing, dressing, grooming, toileting) in accordance with Individual Resident Service Plan, using safe, courteous and effective standards of practice.
Make appropriate observations about resident’s condition, record changes in a timely manner, and report changes to the Health & Wellness Director, Nurse or designated charge person immediately.
Record omission of resident services/medications according to company policy/procedures and standard.
Assist in recording temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output as directed.
Promptly respond to resident call lights.
Demonstrate a courteous, polite, and friendly attitude with families, visitors and co-workers.
Administer and manage medication under the supervision and training of the Health and Wellness Director.
Read, write and speak English sufficient to fulfill job requirements.
Make independent decisions and solve practical problems with instructions given in written, oral, diagram or scheduled form.
Listen actively and evaluate social situations objectively, consistently and fairly. Function independently with integrity, flexibility and collaborative spirit.
Comply with community policies and procedures for workplace safety including infection control, emergency response and safety protocol.
Comply with all company policies related to resident rights, abuse prevention, reporting and confidentiality (HIPAA).
Demonstrate knowledge of Employee Handbook and comply with Handbook policies. Perform other related duties as assigned.
