Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.

The Social Worker provides social work services to Hospice patients and families specific to the assignment. This position is responsible for providing clinical services to the terminally ill patient and family, to assist them to cope with the emotional, financial, and practical demands of the illness and the dying process. The Social Worker is responsible for providing interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. They skillfully advocate in empowering patients to maintain a level of control over their treatment and ensure self-determination. In addition, the Social Worker focuses on ensuring quality care while actively engaging in cost effective measures such as Performance Improvements Projects, LEAN activities, and other hospital wide initiatives.

Essential Functions and Responsibilities of the Job

Provides psycho-social assessment and intervention to Hospice patients and their families

Utilize community resources to assist in resolution of personal/financial problems related to terminal illness

Counsels and supports patient/family and advocates to allow for death with dignity and maintenance of optimum family/patient relationships and adjustments

Communicates current social needs/problems/goals at IDT and suggests interventions to Plan of Care

Documents in the EMR timely and concisely

Assist Hospice team members in understanding significant social and emotional factors related to the patients terminal illness and the family’s adjustment to the state of illness

Provides bereavement support; provides Presentation of Benefit as needed

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.

Minimum Requirements Qualifications / Work Experience

Minimum 2 year’s recent experience in a Healthcare setting. Hospice and/or Oncology experience preferred

Acceptance and knowledge of hospice philosophy required

Electronic Medical Record experience preferred

Education / Licensure / Certification

Master’s Degree in Social Work from a school accredited by the Council on Social Work Education

Basic Cardiac Life Support Card

Must possess a valid California driver’s license and vehicle insurance (California minimum)
