Coordinate the referral of patients to home health, honoring patient choice in service selection Evaluate patient’s suitability for home health upon request from the physician or facility Act as a liaison between the patient, caregiver, physician, and home health agency Facilitate the procurement of DME equipment and ensure delivery and special procedures when applicable Provide educational in-services regarding home health services to healthcare professionals What is Required (Qualifications)
Formalized clinical training from an accredited institution Licensed in the state of practice Home Health or Hospice experience Solid computer skills, including Microsoft Outlook and CRM software Excellent presentation, negotiation, and relationship-building skills How to Stand Out (Preferred Qualifications)
