Social Worker (PACE) (Bilingual in Spanish)
– CalOptima, Orange, California Full Office; Telework approved only in California.
About the Opportunity Join CalOptima as a bilingual Social Worker for our Program for All-Inclusive Care for the Elderly (PACE). You will deliver high‑quality, compassionate social‑work services, support the interdisciplinary team, and help shape the future of community health.
Responsibilities
70% – Care Management
Conduct participant screening, case management, counseling and referrals for PACE participants.
Participate in IDT assessments and care planning (initial, 6‑month, annual).
Document changes in the medical record per standards.
Communicate participant status and needs with IDT members.
Assess high‑risk situations (suicidal/homicidal ideation, abuse) and engage in safety planning.
Provide group counseling and support group activities.
Refer to community agencies, act as liaison and advocate for participants.
Assist with advance directives, funeral planning, and financial resource planning.
Address psychosocial needs with appropriate interventions.
Develop discharge plans and coordinate home visits as required (frequent travel in Orange County).
Support transport, family communication, and warm handoff processes.
Advocate for participants’ care needs and develop coping strategies.
25% – Support Services
Assist department in meeting short and long‑term goals.
Consult staff on social and cultural factors affecting health and available community services.
Attend in‑services, staff meetings, and case conferences.
Support enrollment processes and communicate gaps.
Contribute to program improvement and work with QI on grievances, appeals, and advocacy.
5% – Other
Complete projects and duties as designated.
Qualifications
Master’s degree in Social Work from an accredited institution plus 2 years of health‑related experience (or equivalent education and experience).
Minimum 1 year of case‑management experience with elderly populations and experience with elderly within the last 3 years.
CPR and First‑Aid certification required or obtained within 6 months of hire.
Bilingual in English and Spanish (legally required language).
Valid driver’s license, vehicle or approved transportation; clean driving record needed for work away from primary office 30%+ of time.
Preferred Qualifications
1 year of case‑management experience specifically with the elderly.
Knowledge & Abilities
Ability to build rapport and maintain effective relationships with staff, leadership, and external contacts.
Independent work, sound judgment, clear written and verbal communication.
Schedule flexibility; availability for evenings and weekends.
Project management, analytical, organizational, and problem‑solving skills.
Leadership in multi‑program teams and external committees.
Proficiency with Microsoft Office and job‑specific systems.
Physical Requirements
Visual acuity to read screens and documents.
Clear verbal communication, hearing, manual dexterity for typing and standing.
Ability to bend, crouch, walk, kneel, and prolonged sitting.
Lift and move objects up to 25 pounds.
Compensation & Benefits
Pay Grade 309 – $68,015 to $108,824 (based on experience and equity).
Full‑office position; telework eligible in California only.
