Team Lead Home Care MedStar Health is looking for a Team Lead Home Care to join our team. The Team Lead is responsible for facilitating physician‑ordered home health care arrangements and supporting staff in the coordination of services. This role serves as a key liaison between the agency and hospital case management teams, department managers, and directors, fostering strong working relationships to ensure smooth transitions of care.

Under the direction of leadership, the Team Lead promotes consistency by enforcing standardized care coordination practices across the team. They provide hands‑on training, monitor staff productivity and quality, and coach team members to enhance performance and service delivery.

The Team Lead supports efforts to increase hospital referrals by assisting with in‑services, homecare displays, and participation in hospital meetings and outreach activities. They maintain a high level of customer service at all times and act as a resource for both internal staff and external partners. Additionally, the Team Lead provides referral assistance and departmental coverage as needed, stepping in for the Manager, Supervisor, or other Team Leads to ensure continuity of operations and support.

Primary Duties

Achieve or exceed MHHC/MHHI admission goals through active referral development, managing an average of 40 referrals per month, identifying new referral opportunities, assisting with MedStar site coverage, and collaborating with leadership to ensure effective resource allocation.

Lead and support the Clinical Consultant Team by coordinating daily referral activities, contributing to performance evaluations, managing time‑off calendars and weekly reporting, assisting with complex cases, and ensuring appropriate coverage and regional planning.

Assess physician and referral source educational needs, develop and deliver in‑service presentations, support team education initiatives, and provide orientation, onboarding, and site‑specific training for new Clinical Consultants.

Partner with management and Human Resources to recruit, interview, and onboard Clinical Consultants while fostering strong relationships with physicians, referral partners, and internal departments to support coordinated care delivery.

Participate in multidisciplinary quality and service improvement initiatives, represent the department in meetings, committees, and community outreach efforts, and support ongoing professional development and organizational goals.

Qualifications

Bachelor’s degree (Required)

1–2 years experience in social work, in a hospital setting (Preferred)

1–2 years experience in home care (Preferred)

License/Certifications

RN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia, Maryland or Virginia or any combination as required based on work location(s) - Upon Hire (Required)

APTA - Physical Therapy in the District of Columbia, Maryland, or Virginia, or any combination as required based on work location(s) - Upon Hire (Required)

AOTA - Occupational Therapy in the District of Columbia, Maryland, or Virginia, or any combination as required based on work location(s) - Upon Hire (Required)

LMSW - Licensed Medical Social Worker - State Licensure in the District of Columbia, Maryland, or Virginia, or any combination as required based on work location(s) - Upon Hire (Required)

Pay Range USD $74,214.00 - USD $134,596.00 / Yr.
