As a Hospice CNA, you'll play a vital role in bringing comfort, peace, and reassurance to patients in the place they call home. Location: Grand Rapids Schedule: MondayFriday | Day Shift | No weekends What You'll Gain Flexible scheduling with the autonomy to manage your day Competitive pay, mileage reimbursement, and performance incentives Comprehensive benefits including medical, dental, vision, 401(k), life and pet insurance Generous paid time off, holidays, and bereavement leave Tuition reimbursement and opportunities for career growth A supportive, collaborative team culture grounded in compassion and excellence What You'll Do Deliver hands-on, compassionate care aligned with each patient's individualized care plan Support patients and families with dignity, comfort, and emotional presence during end-of-life care Assist with personal care and educate patients and caregivers on self-care techniques Communicate patient updates and changes in condition to the RN Case Manager and care team Accurately document care provided and patient needs Help prevent unnecessary hospitalizations by supporting continuity of care at home What We're Looking For High School Diploma or GED At least 1 year of experience as a Nursing Assistant or Hospice Aide Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR 484.36) Hospice or home health experience preferred Reliable transportation, valid driver's license, and auto insurance (approximately 50% travel) Ability to lift 50100 lbs and perform hands-on caregiving duties Preferably resides within the service area Be Part of Something Meaningful At Elara Caring, you're more than a CNAyou're a source of comfort, strength, and peace for patients and families when they need it most.
