POSITION SUMMARY: The Community Relations Liaison, in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of the organization to current and potential referral sources, patients, their families, and the community at large. The Liaison will provide education to patients, families, caregivers, and facility staff regarding patient progress and other relevant areas to ensure high-quality care. REPORTS TO: Director of Operations or Executive Director. SUPERVISES: None. QUALIFICATIONS: Education: Bachelor’s degree in a healthcare-related field preferred. Experience: 2+ years in hospice, palliative, home health, or similar fields preferred. Core Competencies: Knowledge of Medicare, Medicaid, and Private Insurance regulations; excellent communication skills; ability to observe and report patient status; experience in marketing strategies. Other: Valid driver’s license and current automobile insurance. FUNCTIONS & RESPONSIBILITIES: Work with the admission team and establish organizational presence within the community. Assist with obtaining admission documents and financial information. Respond to referrals and explain the program to patients and referral sources. Support referral development and marketing efforts. Participate in meetings, complete documentation accurately, and comply with policies and regulations.
