Key Responsibilities
Support the needs of Individuals (both minors and adults) by coordinating services, including initiating and completing intake assessments, interviewing prospective individuals and families, organizing schedules and appointments.
Maintain open lines of communication with key stakeholders (Individuals, parents, conservators, agencies, employers, etc.) as you complete daily case notes, quarterly progress reports, and share recommendations to the individual’s Service Plan.
Advocate for the Individual in court proceedings as part of the Judicial Intervention and Diversion Compliance Program.
Assist in building community partnerships to enhance available resources for the Program, improving access to community, physical, and mental health services.
Participate in all required training, team, and company meetings to grow your knowledge and skills.
Take ownership of your own behaviors and help coach others to demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, empowerment).
Receive ongoing coaching, performance feedback, and be recognized for personal and team contributions while embodying the C.A.R.E. values.
Required Qualifications
High School diploma or GED equivalent and at least 18 years of age.
Bachelor’s degree in psychology, behavioral science, social services, or related discipline or four (4) years of relevant experience working with individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement.
Current driver’s license and California automobile insurance coverage for driving a personal vehicle to and from various community locations independently or with team members/Individuals.
Ability to read, write, and speak English is required; bilingual in English/Spanish is preferred.
