Full-time (Monday–Friday, 10:00 AM – 6:00 PM)

We help individuals achieve stability and independence through housing supports, skill-building, and inclusion services funded by NJ.
Position Summary We’re seeking a compassionate and organized
Housing Specialists in Bergen, Essex, Union, Hudson County
to assist clients in securing and maintaining safe, affordable housing. The Housing Specialist will coordinate with clients, landlords, and managed care providers to ensure successful tenancy, community integration, and compliance with program requirements.
Key Responsibilities
Conduct housing assessments and determine client eligibility for housing caseload of 20 clients per month in person.
Assist clients with housing search, applications, and move-in coordination
Build relationships with landlords and local housing programs
Support clients in maintaining lease compliance and resolving tenancy issues
Provide ongoing tenancy-sustaining services (budgeting, life skills, advocacy)
Collaborate with MCO housing teams and Grace Community Care’s intake and billing departments
Maintain accurate documentation and case notes in Microsoft.
Participate in regular team meetings and training
Qualifications
Experience in
case management
and
healthcare
required.
Knowledge of
NJ familycare/medicaid, DDD, or Section 8
preferred
Strong communication and organizational skills
Valid NJ driver’s license and reliable transportation
Ability to travel 100% locally for home visits or landlord meetings.
Must have your own computer and highspeed internet.
Ability to meet with clients at a safe library.
100% field visits required.
