Tribal Option Care Manager – Float
Position at Cherokee Indian Hospital. The Care Manager performs care management for individuals of all ages, ensuring comprehensive, person‑centered care planning and coordination of services across medical, behavioral, and social support systems.
Responsibilities
Complete Care Needs Screenings (CNS) and Comprehensive Assessments (CA) for eligible members.
Develop care plans/ISP based on member needs and desires, coordinating with multidisciplinary teams.
Coordinate referrals, follow‑up on admissions and discharges, and facilitate transitions to community placements.
Provide member education and counseling on health assessment, disease processes, medication management, and community resources.
Use electronic health records and care management platforms for documentation and monitoring of health indicators; interpret data to identify gaps.
Implement the Healthy Opportunities Pilot (HOP) program for eligible members and manage 1915(i) service requirements.
Ensure robust medication reconciliation, support adherence, and maintain detailed communication with providers and family.
Assist with on‑call duties, callback, and occasional administrative leave to safeguard member care.
Qualifications
Minimum of two years of nursing or related experience.
Unrestricted Registered Nurse license in North Carolina or a reciprocal‑accepted state.
Basic Life Support (BLS) certification; may be obtained within 6 months of hire.
Valid North Carolina driver’s license.
Specific experience working with Native Americans is preferred.
Strong interpersonal, communication, and conflict resolution skills.
Proficiency with Microsoft Office and care management platforms.
Ability to independently plan, prioritize, and make decisions under pressure.
Education & Experience
Bachelor’s or Associate’s degree in Nursing (or related health field such as psychology, sociology, social work, or special education).
Job Knowledge
Planning, managing, and organizing work to meet priorities and deadlines.
Understanding of multidisciplinary health teams and cultural context of the member population.
Knowledge of care management processes: screenings, assessments, care plan development, motivational interviewing, and community resources.
Understanding of Medicaid and other benefit plans, as well as state and federal regulations.
Proficiency in reviewing data for person‑centered decision making.
Complexity of Duties Must apply independent judgment when selecting the most appropriate guidelines and regulations for each member and situation.
Supervision & Responsibilities for Accuracy Works under the supervision of the HOP Supervisor, Assistant Director of Care Management, and Director of Care Management. Responsible for maintaining accuracy in policies, procedures, and documentation.
Interpersonal Skills & Legal Compliance Maintains confidentiality in compliance with the Privacy Act of 1974, employing professional conduct and sensitivity when interacting with members, families, and healthcare providers.
Physical & Working Conditions Work is predominantly sedentary, with occasional walking, standing, lifting, and exposure to communicable diseases. Requires adaptability to a clinic setting with moderate risk of exposure to infectious disease, radiation, and chemicals.
