Make a Difference
The Life Skills Specialist (LSS) monitors and provides care coordination and life skills training and development in collaboration with the care team to youth and/or adults with mental health and/or substance use disorders, including those with serious and persistent mental illness and chronic addiction. Exceptional Skills And Qualifications
Collaborates as part of a multidisciplinary care team; assists with gathering data for comprehensive psychosocial assessments; able to deescalate clients displaying aggressive behaviors; documents services provided in Electronic Medical Record (EMR) based on policies and procedures; when necessary, will transport clients in personal vehicles. Complies with all applicable Behavioral Health Care Service policies and procedures; actively involved in professional development and education, including mandatory training; completes time cards, PTO requests, mileage reports, and other documentation per Community Behavioral Health policies and procedures. Individual may not perform all of these duties, or may perform additional, related duties not listed here. High School Diploma or GED (required) Current Indiana Driver's License (Required) Two (2) years of social service experience in a volunteer or paid role (required) Apply today for immediate consideration.
