We are seeking a Hospice Community Liaison to expand community awareness and strengthen referral partnerships. A confident communicator who can build trust with healthcare partners
A track record of meeting or exceeding outreach or sales goals
Understanding of healthcare compliance and industry standards
Professional, respectful, and collaborative
Well-established network within Houston and surrounding areas (preferred)
Key Responsibilities
Develop and nurture referral relationships with physicians, clinics, senior care communities, and other healthcare partners
Conduct outbound calls and in-person visits to increase visibility and generate referrals
Use market insights to identify growth opportunities and new referral sources
Deliver presentations and educational in-services on hospice services and care outcomes
Partner with internal teams to ensure a smooth referral-to-admission process
Represent the organization at community events, activities, and outreach programs
Provide daily updates and activity reports to leadership
Proactively resolve barriers to care and ensure timely follow-up for referrals and admissions
