Peer Support Specialist Job Summary The Peer Support Specialist (PSS) provides experience, strength and hope to individuals, their families and significant others seeking care. The PSS helps minimize the emotional impact of treatment, increases the likelihood of recovery, and supports continuity of care after discharge. The role includes coordinating transition to the most appropriate level of care, ensuring coordination with all parties involved, and engaging family and social supports in discharge planning. The PSS works as part of a multidisciplinary team comprising a Registered Nurse (RN), Patient Care Technician (PCT), Behavioral Health Clinician (BHC) and supervisory/administrative staff.

Responsibilities

Provide peer support and emotional encouragement to patients during treatment and transition periods.

Coordinate with the treatment team to facilitate smooth transitions to appropriate care levels.

Ensure integration of family and social supports into the discharge planning process.

Collaborate with RN, PCT, BHC and administrative staff to align care goals and discharge plans.

Qualifications & Requirements

High school diploma or GED required; Associate Degree or higher preferred.

Minimum of six months of recovery from substance use and/or mental health disorders required.

Health‑care related experience that provides exposure to behavioral and medical crisis situations preferred.

Valid identification required.

Certified Peer Support Specialist certification required for Screening and Assessment programs.
