Summary: This Senior Case Manager has experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized practice in the Senior Social Worker's professional discipline. The Senior Social Worker is licensed or certified by a state area of specialized professional practice.

Responsibilities

The HUD-VASH program is in multiple sites within the catchment area of NCHCS.

Supervision of the Senior Social Worker in providing case management for a caseload of Veterans in the HUD-VASH program.

Reviewing agreements between Section 8, landlords, Public Housing Authorities, and the VA to establish appropriate referral sites for HUD-VASH placements.

Performing site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.

Verifying the Veteran's status, eligibility for VA medical care, and eligibility for program participation.

Ensuring that homeless Veterans are considered as primary participants for program acceptance.

Collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.

Providing HUD-VASH case management and coordinating care, including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.

Qualifications

United States Citizenship; selective service registration required for males born after 12/31/1959.

Proficiency in written and spoken English.

Master's degree in social work from a school fully accredited by CSWE; doctoral degree not substituted.

Licensed or certified by a state to independently practice social work at the master's degree level.

At least two years of experience post advanced practice clinical licensure, with one year at the GS-11 grade level.

Experience in a specialized area of social work practice and advanced practice skills and judgment.

Possession of specialized interventions, treatment modalities, and advanced case management skills.

Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.

Knowledge in developing and implementing outcome evaluation methods and improving services.

Ability to provide specialized consultation, role model effective social work practice skills, and expand clinical knowledge.

Valid driver's license; periodic patient transportation may be required.

Demonstrated knowledge, skills, and abilities as described (E.g., specialized consultation, policy writing).

Driver's licensure requirements: periodic transport, CA DMV program review.

Work Schedule: Full Time, Monday - Friday 8:00‑4:30.

Pay: Competitive salary and regular salary increases; a higher step rate may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (above minimum rate of the grade).

Paid Time Off: 37‑50 days of annual paid time offer per year (13‑26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full‑time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement: Traditional federal pension (5‑year vesting) and federal 401K with up to 5% in contributions by VA.

Insurance: Federal health/vision/dental/term life/long‑term care (many federal insurance programs can be carried into retirement).

Telework: May be ad‑hoc available.

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools in candidacy status do not meet this requirement until the school is fully accredited. A doctoral degree may not substitute for the master's.

Licensure: Persons hired or reassigned must be licensed or certified by a state to independently practice social work at the master's degree level. Senior social workers must be licensed or certified by a state at the advanced practice level, unless grandfathered.

Demonstrated Knowledge, Skills, and Abilities: Skill in a range of specialized interventions and treatment modalities. Ability to incorporate complex differential diagnosis and treatment. Knowledge in outcome evaluation. Ability to provide specialized consultation, role model effective practice, expand clinical knowledge, and write policies, procedures, and practice guidelines.

Driver's Licensure: This position involves periodically transporting patients and requires a valid driver's license. The employee's driver's record will be reviewed regularly through the CA DMV Employer Pull Notice (EPN) Program.
