REACH Residential Coordinator **SIGN-ON BONUS ELIGIBLE**

Richmond Behavioral Health is seeking a full‑time REACH Residential Coordinator. The selected candidate will perform intermediate‑skilled human support work, providing clinical and coordination services as part of the REACH Home (REACH CTH or Transition Home). The role involves coordinating client support in financial, housing, medical, psychiatric, and social needs, maintaining client safety and overall stability in community living, and related duties.

Essential Functions

Implements REACH team support for individuals with intellectual and developmental disabilities and intense behavior needs.

Models therapeutic groups and implements therapeutic curricula at the REACH Home for REACH staff.

Maintains communication and relationships with community partners.

Completely conducts Waiver Residential Crisis Supports assessments and reassessments for individuals at the REACH Home.

Develops and revises treatment plans for individuals admitted to the assigned REACH site (residential).

Reviews and/or develops safety plans for each REACH admission; revises as needed during the person's admission.

Ensures the coordination of treatment team meetings each week and facilitates meetings.

Participates weekly in Multi‑Disciplinary Team (MDT) meetings and provides clinical updates on all current guests.

Ensures MDT recommendations are incorporated into current treatment delivery and discharge planning / discharge instructions.

Possesses strong case conceptualization skills.

Completes Crisis Education Prevention Plans (CEPPs) as needed.

Participates in required meetings with REACH Home Team and Supervisors.

Identifies and coordinates necessary training and technical assistance needs for REACH staff, community providers, natural supports, and other components of the system of care to help prepare them for the REACH Home guest's step‑down back to the community.

Links individuals to additional supports and services as recommended.

Completes REACH‑sponsored/required trainings on an ongoing basis.

Travels to off‑site appointments/meetings when necessary.

Completes documentation in the electronic health record in accordance with collaborative documentation, Licensing, DMAS, and agency requirements.

Participates in the leadership on‑call rotation to include providing crisis support/coverage to the REACH CTH or Transition Home and/or working shifts as needed (days, nights, weekends and holiday rotation).

Ensures that behavioral data is being collected, analyzed and reported on routinely for all ATH residents with behavioral support needs.

Provides training and consultation to the residents' systems, including families and providers.

Utilizes behavioral and clinical documentation and data to develop and update behavioral profiles on all ATH residents.

Ensures required trainings are current and completed in a timely manner.

Performs other related work duties as assigned.
