Position Summary This individual will serve as a liaison between KSOSN, partner facilities, dialysis organizations, transplant, and surgeons to successfully assist patients in having an optimal dialysis start, being placed on the transplant list and receiving care and education. The Care Navigator will be committed to creating a quality environment, promoting the patient experience and patient safety, creating an exceptional work experience for our clinical team and patients. The Care Navigator will partner with the leadership team to achieve KSOSNs goals.
Essential Job Functions
Serves as the primary contact for the initial administration of patient referrals and admission to dialysis center for KSOSN providers, vascular access placement and transplant list placement
Acts as a liaison with the referral sources, centers, patients, and discharge planners; obtains medical information in a timely manner to ensure an expedited and efficient placement process to a preferred center location
Responsible for maintaining the dialysis census, dialysis reports and database vascular access and transplant placements
Uses knowledge of KSOSN, patient, and referral source locations for proper navigation
Schedules high priority patients for continuous health examinations.
Responsible for following the patient admission process, patient transplant placement and vascular access placement; communicates to appropriate parties according to the established procedures and in compliance with all regulatory requirements
Collects all data or packet for placement according to KSOSN policy to ensure efficient processing of placement, including insurance and other financial information; completes the preliminary paperwork and standard placement forms including assisting with collection and documentation of comorbid information
Obtains necessary approvals to ensure the efficient processing of placement data according to KSOSN policies and procedures
Utilizes appropriate systems to ensure accuracy, tracking, and reporting
Coordinates internally and externally with Intake/Authorizations to establish appropriate placement of patients
Follows up in a timely manner, and escalates urgent needs such as placement denials from centers or billing groups to management as needed
Communicates in a timely manner with individual centers based on placement and prompt scheduling processes
Collaborates with center staff and other internal departments to effectively resolve issues impacting a patient’s placement process
Recommends updates or changes to the admission criteria, policy, and procedure
Maintains effective and professional relationships with all referral sources by providing outstanding customer service
Develops and maintains effective relationships through effective and timely communication
Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
Other duties as assigned.

Qualifications Minimum Requirements
High School Diploma required
Minimum three (3) years of related experience in a fast-paced healthcare environment that includes familiarity with contract and network benefits.
Must have intermediate knowledge of Microsoft Office applications
Must possess excellent customer service, communication, organizational and interpersonal skills
Must be detail oriented with data entry proficiency
Must have the ability to adapt to change and support new policies and procedures in a positive manner
Must be able to read, write, speak, understand and satisfactorily communicate with others in English in person, over the phone and via email
Associates Degree preferred; and Bachelors Degree strongly preferred
Spanish a plus
