Job Summary We’re hiring a field-based Community Outreach & Referral Marketer to grow our private‑pay home care business through relationships and referrals. This role is about being in the community, meeting referral partners, building trust, and converting referrals into started care hours.
Important: This is NOT a social media/content creator role. This is community outreach / business development.
What You’ll Do (Key Responsibilities)
Build and maintain referral partnerships across Broward County (ALFs, hospitals, rehab/SNF, physicians’ offices, case managers/discharge planners, elder law attorneys, CPAs, senior communities, community organizations).
Execute consistent face‑to‑face outreach: introductions, follow‑ups, relationship nurturing, and scheduled meetings.
Own a weekly territory plan: target list, visit cadence, and pipeline coverage.
Track all outreach activity and outcomes in a CRM or tracker (visits, meetings, referral sources activated, referrals received, conversions, next steps).
Coordinate internally to ensure fast response and excellent follow‑through on referrals.
Represent the agency at networking events, professional meetings, and community health fairs.
Collect market intelligence (competitors, referral trends, partner needs) and report insights to leadership.
How Success Is Measured (Scorecard)
Billable hours initiated from your referral sources (weekly/monthly)
Referral partner meetings completed (weekly)
Active referral sources producing referrals (monthly)
Conversion rate: referral → assessment → started care
Pipeline coverage: scheduled follow‑ups and next steps logged
Compensation
Base salary + commission based on performance measured in billable hours initiated (commission plan details shared during the interview).
Mileage reimbursement / car allowance
Requirements
Comfortable doing field outreach and face‑to‑face relationship building daily
Strong follow‑up discipline, organization, and communication
Reliable transportation and ability to travel throughout Broward County
Basic ability to use CRM/tracking tools (or learn quickly)
Professional appearance and confidence speaking with healthcare/senior‑care stakeholders
Preferred Qualifications
Experience in home care, senior services, healthcare outreach, hospice liaison, DME/medical sales, or B2B relationship sales
Existing referral relationships in Broward County (nice‑to‑have, not required)
Bilingual (English/Spanish) (optional / preferred)
